Individual Support Coordinator

Fairfield DD seeks a full-time Individual Support Coordinator. This position is responsible for individualized service and support administration functions for designated individuals. The chosen candidate will also assist individuals in identifying choices, preferences and strengths through a self-determination planning process where the individual, family members and/or personal advocate are in charge. This ISC will coordinate individual assessment of strengths, needs, concerns, priorities, hopes and interests for adults or children.
 
As an employee of the Fairfield County Board of Developmental Disabilities, the job incumbent shall comply with all Board policies, and shall demonstrate respect for, support dignity of, and observe the rights of all individuals served by the agency.
 
The starting hourly rate for this position is $22.81 and is based upon experience and qualifications and includes full county benefits. A Bachelor’s Degree is required for this position.
